Liability Photo Waiver

I, , am a parent or guardian of the child(ren) who will be attending the Renaissance Academy. In consideration of the child(ren) being permitted to attend the Renaissance Academy, I acknowledge, understand and agree that: (1) the risk of injury from the child(ren) attending the summer camp is a possibility; (2) I KNOWINGLY AND FREELY ASSUME ANY RISKS from the child(ren) attending the Renaissance Academy Summer Camp both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE of those persons released from liability below and assume full responsibility for the child(ren)'s participation in summer camp activities; (3) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS THE RENAISSANCE ACADEMY, ITS TEACHERS, ADMINISTRATORS AND OTHER EMPLOYEES ("Releases"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to any person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE; and (4) I understand and agree that this Release of Liability Agreement covers each and every activity at the Renaissance Academy.

I, do hereby release to Renaissance Academy, its assigns, licensees, affiliates and legal representatives the irrevocable right to use my picture, images of my art, portrait, photograph or video image in all forms and media and in all manners, including composite or distorted representations, for any and all purposes including advertising in all forms, for unlimited time, and I waive the right to inspect or approve the finished product, including written copy, that may be created in connection therewith. I have read this release and am fully familiar with its contents.

In case of emergency, the parents/guardians understand that every effort will be made to contact them. In the event that they cannot be reached, they hereby give permission to the Renaissance Academy Staff to order any necessary procedure and to secure proper treatment for the health of the students.

As per NJ Department of Health Requirement, camper must submit the immunization records and/or religious examptions.

Refund Policy:

Registration Fee, Late Fee are non-refundable

The rest of fee are fully refundable 2 months before the start of summer camp

If withdrawn by 3rd weeks before the summer camp starts, 75% of summer camp fee is refundable

If withdrawn by a week before the summer starts. 50% is refundable

No fee is refundable after the summer camp starts
